Select “Manage or make changes to an individual disability insurance policy.”

Complete and print the form titled "Set up or
Change your Automated Payment Information (EFT)".

In a few days, you’ll receive a confirmation letter letting you know when your next payment will be withdrawn. Plus, we’ll send you a communication before we withdraw money for your payment (except if you select monthly payments).
