Effective Monday, Sept. 14, 2020, we can only send out an electronic policy when we have authorization from the customer to do so. The Electronic Signature and Delivery Disclosure form (UN 2076) has been updated and is available on Producer Workbench. This form will automatically download with all application packets for eligible life, disability and annuity products. The updated version of the form is also available on eApply.
